Household Enquiry Form
Every household in Derbyshire Dales receives a Household Enquiry Form annually, at the beginning of August, addressed to The Occupier.
By checking and confirming or amending the information on the form you will help us to ensure that our Electoral Register is kept up to date.
EVERY HOUSEHOLD NEEDS TO RESPOND
The table, below, shows how you can send us the information we need. By providing this information promptly online at www.registersecurely.com/derbyshiredales or by phone 0800 316 1570 (for no change only) you will help us to minimise return postage and the need to send reminders and/or a personal canvasser, all of which add to the cost of the process that, ultimately, falls on the council tax payers.

The two passwords you will need to enter if completing your Household Enquiry Form online or by phone can be found in the box on the front page of the form that we send to you.
IF YOU ADD SOMEONE TO THE FORM
Adding a name to the form does not mean that they will automatically become registered to vote. They will also need to make an individual application and can do so at www.gov.uk/register-to-vote They will need to have their national insurance number handy when doing so. If they do not apply on line we will send them an individual application form in due course.
There is a comment box at the end of the online form where you can note any queries, requests or additional information you may wish to share with us. This will be monitored regularly and we would be grateful if you could include contact details.

Please complete and return the form, before the deadline shown, in the prepaid envelope provided.
NB: You do not need to return the form if you have responded online or by phone.
