In the Derbyshire Dales the Gold Standard Health Referral is a partnership between the District Council and Derbyshire County Primary Care Trust. Referral is via a local health professional and referral will be to one of our Active Health Referral Instructors.
Active Health Referral is a service designed to encourage people living with a diagnosed and specific medical condition, to take part in physical activity on a more regular basis to improve their health and quality of life. A range of activities will be available and activities will be tailored to individual needs and preferences by a dedicated Active Health Referral Instructor. The service also includes patients who have suffered a heart attack and have been referred by their hospital
