Active Health Referral

Derbyshire Gold Standard Health Referral Service:

In the Derbyshire Dales the Gold standard Health Referral is a partnership between Derbyshire County Primary Care Trust and Derbyshire Dales District Council. Referral is via a local health practitioner and referral will be to one of the Council's leisure facilities. 

Health Referral is a service designed to encourage people living with and/or at risk of a medical condition, to take part in physical activity on a more regular basis to improve their health and quality of life. A range of activities will be available and activities will be tailored to individual needs and preferences by a dedicated Active Referral Officer.

Gold standard Health Referral supports the Derby and Derbyshire Obesity Strategy, Choosing Health priorities (obesity, mental health, inequalities), Local Area Agreement (LAA) targets, National Service Framework standards for health improvement and National Institute for Clinical Excellence (NICE) guidelines.
